AFFIRMATION By proceeding with this transaction, you acknowledge that: 1. If you are an individual donor, you affirm that you are a U.S. citizen or lawful permanent resident and have not knowingly or willfully accepted funds, directly or indirectly, of more than $10,000 from one or more prohibited sources* within four years of making this donation. 2.
